Denial Code PR 177 - Eligibility Requirements Unmet

Code 177 indicates that the claim was denied because the patient has not met the necessary eligibility requirements for coverage. This means the patient's insurance plan did not recognize them as eligible for benefits at the time of service.

Who Pays: Group Code Liability

If code 177 is accompanied by group code PR, the patient is responsible for the charges and can be billed. However, if the group code is CO, the provider must write off the amount, and the patient cannot be billed.

Why Claims Get Code 177

  • Patient's insurance coverage was inactive on the date of service.
  • Incorrect member ID was submitted, leading to an eligibility mismatch.
  • Patient changed insurance plans and the update was not reflected in the system.
  • Coordination of Benefits (COB) was not updated, causing eligibility issues.
  • Patient did not meet plan-specific eligibility criteria, such as age or service area.

How to Fix & Resubmit

  1. Verify the patient's insurance coverage was active on the date of service.
  2. Check that the member ID and other patient details were accurately submitted.
  3. Contact the patient to confirm any recent changes in insurance plans.
  4. Review coordination of benefits to ensure primary and secondary insurances are correctly assigned.
  5. If eligibility was incorrectly denied, resubmit the claim with corrected information.

Corrected Claim or Appeal?

Submit a corrected claim if there was an error in the details provided, like member ID or insurance plan. If the patient truly was ineligible, an appeal is not appropriate.

Preventing Future 177 Denials

  • Always verify patient eligibility and active coverage before services are rendered.
  • Ensure patient information, especially member ID, is accurately entered into the system.
  • Regularly update insurance information to reflect any changes in patient coverage.
  • Conduct periodic audits of patient eligibility checks to catch systematic errors early.
